Cardiovascular Surgery

1. Mrs. Rukiya Bokari, a 73-year old female was admitted to NMC Specialty Hospital, Dubai with complaints of chest pain, breathlessness and palpitation. She was a diabetic, hypersensitive and she was admitted in ICCU. She was stabilized and was taken for a coronary angiography. The coronary angiography four vessel disease with compromised LV function (45%). She was advised coronary bypass surgery as the best option. She was taken for coronary bypass surgery. Four bypass surgery grafts were done, including left internal mammary artery. The right coronary endarterectomy was done in toto and a venus graft was anastamosed to the right coronary artery. She made an eventful postoperative recovery and was discharged on the 12th day.

2. Mr. Mohammed Shadid, a 65-year old male IHD, DM, HT, underwent coronary angioplasty in 1995 and 1998. He came to us with complaints of chest pain on exertion, he was stabilized and stress test was done, which was found to be positive. Subsequently coronary angiogram did showed LAD proximal stent block and new disease in the right coronary artery. The best option was a coronary bypass surgery. He was taken for a coronary bypass surgery. The surgery was performed off pump and on beating heart. He left mammary artery was anastamosed to left anterior descending artery and a vein graft was anastamosed to the posterior descending artery. He has a smooth post-operation recovery and was discharged on the 9th day of the operation. This was a beating coronary artery bypass surgery on a 65-year-old male who was suffering from Diabetes, hypertension and two previous angioplasties.

3. A male patient, 55-year old, Diabetic, HT came to me with complaints of dysponea on excretion on and off. He was investigated in Dubai and Mumbai. His Cardiologist, who was following him for the last 10 years, found that his aortic regurgitation has increased to III / TV and his LV function has dropped from 50 % to 40% and was advised early aortic valve replacement. He was scheduled for an elective aortic valve replacement and a bileaflet, mechanical, No: 23, ST.Jude valve was sutured to the aortic annulus by continuous suturing technique using 2-0 Prolene. He made an uneventful post-operative recovery and will be discharged soon.

These are some of the interesting cases we have performed in our four-month old Cardiovascular Department.
